2. Recent pp size photo-1

3. National Identification Card / Document

4. Bank voucher / Online Payment Receipt

5. SEE Grade-sheet or equivalent

6. Transcript of +2 or equivalent

7. Recommendation letter/s for reservation quota

8. License from concerned council (if applicable)

Note:

1. Date in voucher should be in B.S

2. If student has passed class 6-10 from government school then s/he should get recommendation letter from school and education wing of VDC / Municipality office.
